Incoming Inspection Is Not Product Validation
SITUATION
After a pilot build, a water-exposed paper component failed its required tensile behavior. The incoming raw paper had already passed inspection, yet the finished component became harder, absorbed water differently, and did not separate as intended after processing. The raw-material result and the real product result were no longer equivalent.
TASK
I needed to contain the functional risk, determine where material behavior changed, and establish a qualification method that represented the final application—not merely the condition in which the material arrived at the factory.
ACTION
- Contained by Traceable Lot: Segregated the affected material by lot and pallet so the investigation could proceed without mixing evidence.
- Tested the Transformation Chain: Compared raw, intermediate, and finished samples after cutting, printing, lamination, stacking, aging, and water exposure.
- Qualified the Application State: Required water absorption and tensile performance to be verified on a configuration representative of the actual product.
- Controlled Material Substitution: Selected an alternative material route only after equivalent application-state testing, rather than relying on supplier paperwork alone.
- Closed the Governance Loop: Added lot traceability, application testing, material-status control, and release evidence to the PFMEA, control plan, and work instructions.
RESULT
The affected configuration was prevented from moving forward without representative validation, and a suitable material route was established for subsequent production. The enduring improvement was a new release gate: incoming approval confirms what was delivered, while application-state testing confirms whether it is fit for use.
